An employee was providing bedside care to a patient with a catheter and was walking away from the patient when she tripped over the catheter, which was implanted into the patient at the time. In order to avoid forcible removal of the catheter from the patient, she attempted to adjust her fall. In the process of doing so, she landed awkwardly on the floor, resulting in a fracture of her left hip and left wrist. She was hospitalized and required surgery.

An employee slipped and fell while descending stairs, fracturing the left ankle and requiring surgery.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
