An employee was rinsing (spraying water) down a piece of equipment (pump) when he attempted to remove a piece of pipe. He removed a clamp from the pipe, and his finger entered the pipe. A blade (flight) that moves water inside the pipe then amputated his left middle finger at the first joint.
An employee was on a two-step ladder performing cleaning activities in the de-oiler machine. The employee fell back and his right arm was caught in the equipment, resulting in a right forearm fracture that required surgery.
