An employee was using a forklift to load a trailer with a tubular motor. The motor rolled between the forks and fell onto the employee's left leg. The employee sustained multiple fractures to the lower leg.

An employee was changing a leaking valve on a POD with a wrench. While he was loosening the bolt, the wrench slipped off the bolt and his right middle finger was pinched between the wrench and the edge of the equipment, resulting in a fingertip amputation.

A team on the rig floor was attempting to rig down a cement head with a manifold assembly from the rig floor down the slide to the ground below. A winch line was being used to lift the cement head off the rig floor to allow the team members to push the head to the slide. The head was pushed out beyond the slide, but the manifold caught on the rig floor and the head swung back toward the floor and struck the swing gate. An employee was holding the swing gate open with his right hand and was struck on the pinky finger. The distal interphalangeal joint was partially amputated.

An employee was lifting a treating iron to make up a connection. After the connection was secured, the employee started to lower the treating iron when he lost his balance and dropped the treating iron. The weight of the load landed on his right leg, breaking his fibula and tibia.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
