An employee was installing metal edging on roof fascia while working from an extension ladder when he lost balance and fell approximately 18 feet onto a pile of rocks. The employee was hospitalized with a broken pelvis and hip, and also sustained injuries to his knee, elbow, and heel.

An employee was working on the roof of a second story structure, approximately 17 feet from the ground, when he fell off and broke both ankles.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

The employee lost their grip on the drill they were using and the ladder tipped over. An employee fell through a hole in the ceiling and suffered injuries to his side and right arm. When he fell, a piece of concrete fell with him and struck his arm. He was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
