An employee was installing a core on a spindle that was on an extruder. The employee's left little fingertip became caught between the core and an air chuck, resulting in a fingertip amputation.

A maintenance employee was reinstalling the guard after repairing an ink pump when it slipped from their grasp and the employee's left ring finger contacted the moving belt and pulley, resulting in a fingertip amputation.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was operating a laminator and opened an interlocked guard to fix a crease in a web that was running. The employee's right little fingertip contacted a pinch point between the web and the roller and was amputated.

An employee was loading plates into a heat press when they lost balance. The employee went to catch themselves when the press actuated on their hand, crushing their right little and index fingers.

An employee was making a splice on a hanging roll of polyethylene. A forklift bumped the roll, which struck the employee and caused him to fall backward. His back struck a rolling cart and was injured.

The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
