An employee was operating crane hoist machinery. Upon returning the empty cell crane hoist hook to its home position, the employee's left thumb was caught by the hook and partially amputated.

A maintenance employee was attempting to repair a vehicle lift using a scissor lift to gain access to a pulley. When his hand was between the cable and the pulley, the cable realigned, dropped into place, and trapped his left hand against the pulley. He suffered an amputation to the left ring finger below the nail.
