After lining up a cable roller stand with floor anchor holes, an employee was cleaning dirt out of a floor anchor hole. While doing this, another employee moved the cable roller stand to line up a different bolt hole and the injured employee's finger was pinched in a hole. The injured employee sustained a partial amputation and avulsion of the right index fingertip.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
