An employee was walking through the parking lot to their vehicle when they tripped and fell, resulting in injuries to the left knee and right elbow. They were hospitalized.

An employee was trying to clear a jam in a pellet mill. A portion of the mill was still cycling and the rollers caught the employee's right hand. The index finger was medically amputated.

An employee was stepping out of a vehicle toward a fuel pump when his foot slipped off the cement pad of the fuel island and he fell to the ground. He fractured his right leg and was hospitalized.

An employee entered a corn silo to free up a jammed grain sweep when the corn he was standing on gave way, causing him to fall and strike the weights of the machine. He was hospitalized for possible fractured ribs and head trauma.

An employee was using a pry bar to loosen debris on a machine when the pry bar slipped and broke his arm. The machine was not locked/tagged out at the time.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was working on removing the gearbox from the drive shaft of the motor assembly of a feather drier. He was using a hydraulic tool to pull the pieces apart when a bolt dislodged from the eye of the gearbox and the corner of the gearbox broke off. The employee was struck in the nose (sinus cavity), resulting in hospitalization and surgery.

An employee was on a forklift checking the sprinkler system drain. The employee fell 8 feet from the forklift to the floor. The employee suffered a broken collar bone and was hospitalized.

An employee was working as a dumper on a packaging line. A passing forklift turned and pinned the employee against a guardrail. The employee suffered fractures to their ribs and a leg requiring hospitalization.

An employee was cleaning an upper mezzanine. When he leaned against the mezzanine gate, he fell through the gate opening and landed 11 feet below. The employee sustained fractured orbital bones.

An employee was assigned to clean a feed mill in the basement. He then rode a tool and material basket to return from the basement to the ground level. He fell out of the basket and landed about 5 feet below. The employee sustained fractures to his arm, lower back, and foot.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
