An employee was opening the trunk lid of a theme park car when the car lurched forward. The lid then came down on her right ring finger, resulting in an amputation.

He attached a fall protection device to the carriage of the ride and rode the carriage to the top, then stepped off of the carriage and onto an inspection catwalk.
