An employee was opening the trunk lid of a theme park car when the car lurched forward. The lid then came down on her right ring finger, resulting in an amputation.

A maintenance employee was performing daily inspections on the Dominator Ride. He attached a fall protection device to the carriage of the ride and rode the carriage to the top, then stepped off of the carriage and onto an inspection catwalk. He contacted personnel on the ground to lower the carriage so he could inspect the ride cables. He did not disconnect his harness and lanyard and was pulled off the catwalk by the harness and lanyard as the carriage was lowered. He was left suspended in the air from the lanyard and harness, suffering seven broken ribs, as well as neck and shoulder edema and contusions.

An employee was delivering mail when they tripped on a curb and fell to the ground, resulting in a kneecap injury.

An employee was leaving work when they tripped over a curb in the parking lot and fell into a wall, resulting in a fractured left humerus.

A nurse was visiting the home of a patient for requested treatment. She tripped on her gown while going up the steps to the property, falling and fracturing her left femur.

An employee tripped on a step while entering a house. The employee suffered a broken right femur and was hospitalized, requiring surgery.

An employee was walking up an escalator that was out of service when they tripped on a step, fell, and suffered a closed fracture to the right femur as well as a dislocated right patella.

An employee was in a paint booth to change filters underneath the booth. She took a step when her right foot fell into an opening in the floor, resulting in a right ankle injury that required hospitalization and surgery.

On October 11, 2025, an employee was walking through a parking lot when they tripped on a pavement joint and fell, resulting in a fractured right ankle and a left knee injury.

An employee was unloading merchandise from a pallet when she tripped over the pallet and fell forward. Her head and face struck the floor of the stockroom, resulting in multiple head and face injuries that required hospitalization.

An employee tripped on an incline/ramp at the door to a building, fell, and suffered a broken left arm.

At the end of a shift on the downhill coaster, an employee was waiting for an empty cart to arrive at the bottom so he could ride it back up the hill. The cart arrived with its seat belt safety lock engaged. The employee boarded the top of the cart and began working to disengage the lock by reaching under the cart. His right hand and two fingers got caught between the rear wheel of the cart and the coaster rail. His ring and little fingers were crushed and lacerated, resulting in surgery and partial amputation of the ring finger.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
