An employee was changing the saw blade on a piece of machinery. A tool fell onto the machine, causing it to activate, and the blade amputated the employee's middle and ring fingers at the first knuckle.
An employee was holding down the hose pump to transfer chemicals from the dip tank to a tote. The employee was exposed via inhalation to phosphoric acid resulting in chemical burns to the lung.
