An employee was cutting pieces of a conveyor belt when a piece became caught on the saw. The employee sustained lacerations to their left index and middle fingers, resulting in amputation of the middle fingertip.

An employee was exiting a truck when a 200-gallon container rolled behind him and struck him on the back of his leg. The employee sustained a tendon injury that required surgery.

An employee was walking in a customer parking lot when he slipped on ice and fell, hitting the back of his head on the ground. The employee suffered a skull fracture and a herniated disk in his neck.

An employee was using a lift gate to load medical waste containers into the cargo area of a box truck. He fell approximately 4 feet to the pavement, suffering a closed fracture to a lower-back vertebra and a head injury.

An employee was operating the lift gate on his truck to stow it away, when he slipped on ice/water, catching his left thumb on the lift gate. His thumb tip was amputated directly beneath the fingernail.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

On July 24, 2025, an employee was unloading stacks of empty, 96-gallon medical waste containers. A stack of 10 containers, which weighed approximately 460 pounds, fell on him. The employee was hospitalized with multiple head, face, and neck injuries and a back injury requiring surgery.

An employee was standing between a truck and a roll-off container that was being offloaded. The container came down on their left ankle. The employee was hospitalized with a laceration and fracture to the ankle.

An employee was loading a gaylord into a truck when the gaylord ripped, causing him to lose his balance and fall from the liftgate to the pavement. The employee was hospitalized with fractures to his left wrist and femur.

An employee was cleaning the back of a tractor trailer. The tractor trailer pulled away from the dock and the employee fell from the dock lock to the ground below, breaking their right femur.

An employee was preparing a load for the electric pallet jack he was operating. He was walking backward when he tripped and fell. The pallet jack struck his left leg, and he was hospitalized with a broken left fibula.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
