Final narrative
On October 27, 2019, at about 7:45 a.m., an employee was trying to get a power washer to turn over. The employee was testing the battery and solenoid when the motor moved and caught the employee's left ring finger in the belt, resulting in an amputation to the finger.
To prepare to change the oil for a truck, an employee removed a piece of plywood that was covering the pit used for oil changes. The employee fell into the pit, which was approximately 5 feet deep, resulting in a broken femur.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
