An employee was kneeling down to look under an welder machine to see if he could find a bracket that had broken off. The employee lost his balance and his left hand slipped off the frame of the machine. His left index finger was caught between a table transfer cylinder and a metal stop and the fingertip was amputated.
An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.
