An employee was draining liquids from the fluid pot through a ball valve. The employee opened the valve and noticed some debris blocking the port and the employee begin to open and close the valve to clear the debris. Once the debris broke loose, it struck the employee in his left leg causing him to lose his balance and fall to the ground in the direct path of the 920 lbs. of gas pressure and debris. The employee sustained contact burns to his arms as well as contusions to his chest and legs.

An employee was adjusting a blade on a padder machine. As he manually turned a roller, his hand contacted the blade, amputating his right thumb tip.

An employee was cleaning a slicer when the tip of their right index finger was amputated, requiring hospitalization.

The technician was using a prybar to unseat the roller shaft when the prybar slipped, causing him to lunge forward and strike the right side of his face against a valve stem. The technician sustained a fracture and laceration to the cheek, and required surgery for an injury related to his right eye. He was hospitalized.

An employee was cleaning and performing maintenance on a vertical panel saw when the employee impacted the saw blade. They sustained a finger fracture and laceration.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.
