An employee was sanding the pulleys on a ski lift when the employee's hand was pinched between a pulley and the drive belt, amputating the right hand. The employee's head also struck the upper pulley, causing head lacerations and the loss of teeth.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.
