An employee had been standing in the bed of a pickup truck, shoveling salt out to de-ice walkways. After jumping down off the truck he lost consciousness and fell to the ground, striking his head on the concrete. He suffered a head injury and minor brain bleeding, and was hospitalized.
An employee was transferring a resident from a seat to their wheelchair when the back of her sneaker caught on a lip of the floor threshold. The employee fell backward to the ground and the resident fell on top of her. The employee was hospitalized with a fractured left leg and required surgery.
An employee was crossing over a sorting belt and jumped down onto the catwalk next to the belt. The employee sustained a right ankle fracture and dislocation.
