An employee had been standing in the bed of a pickup truck, shoveling salt out to de-ice walkways. After jumping down off the truck he lost consciousness and fell to the ground, striking his head on the concrete. He suffered a head injury and minor brain bleeding, and was hospitalized.

An employee was transferring a resident from a seat to their wheelchair when the back of her sneaker caught on a lip of the floor threshold. The employee fell backward to the ground and the resident fell on top of her. The employee was hospitalized with a fractured left leg and required surgery.

An employee was loading a flatbed trailer when a forklift bumped into the trailer. A wheelset began to roll in his direction and the employee jumped off the trailer to avoid it. The wheelset caught him on the leg and he landed on his shoulder on the ground. He sustained a laceration below the left kneecap and a dislocated shoulder.
