Federal OSHA recorded a severe workplace injury at AMSOIL, INC., 2206 WINTER ST., SUPERIOR, WISCONSIN 54880 on — Skull fracture and intracranial injury, affecting the brain.

On December 17, 2019, at 11:00 AM, two employees were in a scissor lift in an elevated position repairing a natural gas furnace. One of the employees was not contained by the safety gear of the scissor lift and fell approximately 8 feet onto the floor of the warehouse. The employee sustained a fractured skull, fractured vertebrae, and bleeding on the brain as a result of the fall.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

Absolics Inc.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

Berkheimer Maintenance

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

High-Tec Industrial Services, Inc.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

ProLube Oil LLC

An employee was putting together cardboard boxes in order to ship a product while a nearby forklift was grabbing a quarter-full tote from a rack that was 12 feet high. The forklift pushed the tote off the pallet rack and it struck the employee, resulting in broken ribs.

Smitty's Supply, Inc.

An employee was operating a spout machine when a spout became jammed on the spout piston. The employee opened the enclosed plexiglass protective enclosure around the machine to gain access to the jammed spout located on the crimping piston. The employee was unjamming the spout under the transfer table when her right index finger was crushed. The employee suffered an amputation to her fingertip.

An employee was walking when they were struck by a forklift that crushed their right big toe, resulting in surgical amputation.

Amalie Oil Company

An employee was moving 55-gallon oil drums from a pallet on a forklift to a pallet on the floor. One of the oil drums collided with another oil drum, contacting the employee's left middle finger and causing a fingertip amputation.

Bunge Oils

An employee was removing a cap from a pressurized line. The end cap came off the line and struck the employee's jaw, causing a laceration and fracture on the left side.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Asher Enterprises, Inc.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
