Final narrative
On December 21, 2019, at approximately 5:30 p.m., an employee was changing the linens on the bed of one of the members (residents) at the facility. The employee tripped on the cord of the call light button and fell onto the floor. As a result of the fall, the employee sustained a bloody nose and a left arm fracture.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
An employee tripped and fell while walking to a time clock. She suffered fractures to her left shoulder and right kneecap. She was hospitalized and required surgery.
