On February 20, 2025, at approximately 5:45 p.m., an employee was stocking bananas when he tripped and fell to the ground, injuring his right hamstring. He was hospitalized with a hematoma and a torn ligament.

An employee was inflating a tire on a flatbed trailer when the tire exploded. The rim struck the employee, resulting in a broken right arm.

On October 27, 2023, the injured employee was moving material from the aisles to the staging area when another forklift entered the aisle. The injured employee jumped from his forklift, injuring his lower leg on a pallet being transported on the other forklift.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

On September 30, 2025, at 1:45 a.m., an employee was operating a forklift to load materials on an outbound dock. His left foot became caught between the forklift and a pole, resulting in a broken big toe.

An employee was riding a horse in the horse corral/riding arena when the horse bucked multiple times. The employee performed an emergency dismount off the right side of the horse. When she landed, she fell backward in the sand, onto her back. The employee sustained a crushed vertebra.

An employee was pulling an electrical wire from a circuit. The wire contacted a live wire and created an arc flash. The employee sustained burns on their left side and arm.

An employee was operating a heat press machine. His right foot was caught between a paper roller and the heat press, resulting in amputation of a toe.

On 11/08/2024, an employee was working on piping a refrigeration condenser. Refrigeration condensers arrive from the factory with the internal piping pressurized and the end capped. The employee was cutting a capped refrigeration line when the cap blew off, striking the battery-operated saw he was using and knocking the saw it into his face. He was cut under the nose and struck on the head. The angle of the impact pushed metal fragments under his safety glasses and into his eyes. The employee sustained metal fragments in his eyes, lacerations to the face, and contusions to the head.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
