An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. He was helping place and level the blacktop/backfill material over the trench area to bring the road surface back to proper grade. The employee was walking between a forward-moving dump truck and a dirt pile when his right foot was run over by the truck s wheel, causing him to fall. The employee sustained injury to his foot.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

While driving it to the rear of the property to park it, the employee came into contact with a live electrical transformer unit that was located outside of the facility. The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
