An employee was working at a residential home. While ascending a ladder outside, the employee fell around 9 feet to the ground, requiring hospitalization for a possible rib injury and a collapsed lung.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

High-Tec Industrial Services, Inc.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee received a partial amputation to his right middle finger while attempting to secure a load on a tractor-trailer at their facility. Air was released to lower the rollers on the bed of the truck; the employee's right hand was pinched in the rolling slots, causing an amputation to his middle finger.

An employee was in the bucket of a bucket truck, about 16 feet above the ground. A semi truck driving down the road struck the bucket, throwing the employee to the ground. The employee suffered fractures to the leg, elbow, wrist, collarbone, teeth, etc., and was hospitalized.

An employee was trying to stop a truck from driving away from the loading dock with an item left inside the trailer. The employee jumped from the loading dock to the ground about three feet below and fractured the left leg.

An employee located a ticket that required him to maneuver down an embankment under a highway overpass. A contractor then approached the area to have a discussion, so the employee proceeded down the embankment to speak with the contractor. About halfway down, he was stepping on a boulder when it shifted. His right foot twisted and broke, requiring hospitalization and surgery.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
