An employee climbed up stairs to a tank gauging platform, accessed the tank to be gauged, cracked the gauging hatch to let it vapor off while standing upwind, and gauged the tank twice. During the third gauging effort, the employee was exposed to hydrogen sulfide (H2S). He was hospitalized for upper respiratory tract irritation, dizziness and disorientation.

Employee 1 drove an ATV to pick up Employee 2 and then drove to the area that was known to have a natural gas leak in order to inspect the affected area. An explosion/fire occurred in the area and the two employees were severely burned.

An employee was passing through a building when she suffered an allergic reaction to a cinnamon air freshener. The employee began to feel dizzy and ill, ultimately becoming unresponsive. The employee was hospitalized.

While using a flashlight to see, the employee walked into the laundry room and fell into a 2' x 2' access opening for a crawl space in the floor. A black rug was partially covering the opening. The employee fell approximately 40 inches into the crawl space. The employee sustained three fractured ribs on the left side, fluid on the left side of the lungs, and a bruised right knee.

Three employees were changing out a belt on a residual cooling fan when the injured employee's middle finger got caught between the belt and pulley. The employee sustained a fingertip amputation.

Suburban Natural Gas Company

On November 11, 2024, an employee was offloading polyethylene pipe from a truck. The pipe was being moved along free-spinning rollers. The employee's pants got caught on a roller causing them to trip and fall onto a pipe. The employee sustained a puncture wound to their leg requiring hospitalization.

An employee was working on a compressed natural gas trailer gauge when a pressure release struck the employee's chest and forced the employee into a cement post. The employee's back was fractured.
