An employee was walking from the print hut to the main facility building. The employee was found on the ground of the parking lot with a fractured left fibula, two broken left ribs, and a broken left collarbone.

An employee was aligning a rotor shaft to go into a line board. The employee's finger was pinched between the shaft and the board, leading to a fingertip amputation.

Two employees were pushing a multi-tiered material handling cart that weighed approximately 363 pounds. The cart rolled over the foot and ankle of one of the employees. The injured employee sustained lacerations to the ankle and chipped bone.

On 11/4/19, an employee was standing on the bottom row of parts racking approximately 1 foot above the floor to retrieve parts from the top row, which was approximately 5 feet above the floor. At approximately 12:17 p.m., the employee fell backwards from the bottom rack. Her head hit the floor, and the parts bin slid out of the rack and also struck her head. She was hospitalized with a head injury.

An employee was changing a roll on a packaging machine when a roll of paper became hung up on a frame. The employee attempted to free it by pushing on the roll. When it came free, the employee's right index finger was caught between the frame and the roll of paper, amputating it at the first digit just behind the nail.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee was cleaning a machine and bleeding its lines. After the gauge read 0, the employee removed the metal lid covering a filter. The lid popped off and struck the employee in the face, causing a broken nose, a broken jaw, and lacerations.

An employee was burned by an arc flash that occurred in an electrical room.

An employee was carrying a metal plate (18 inches x 18.75 inches) when it dropped onto his right foot, resulting in a fracture.

An employee was troubleshooting a variable frequency drive (VFD). There was an electrical discharge. The employee experienced an electrical shock of approximately 480 volts and sustained second- and third-degree burns on their right hand, wrist, forearm, and elbow.

The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
