An employee was making an adjustment on a case erector when it disengaged, causing a piston to move from the downward to the upward position. The piston pinched the tip of their left little finger, resulting in a fracture and an amputation.

An employee was operating a stand-up forklift and was traveling underneath a mezzanine to drop a pallet of supplies. As they backed out of the area, the mast of the forklift contacted the staircase for the mezzanine, causing the vehicle to stop abruptly. The employee fell out of the lift, resulting in a left elbow fracture and a hip injury.

An employee was cleaning a conveyor belt when his left middle finger was pinched between the belt and a metal housing (lifted so he could clean the belt). He suffered a partial amputation to the finger.

An employee was unjamming a piece of frozen meat from a locked-out auger. When the employee inserted a T-handled bar into the winding down auger, the bar moved, and pinched the employee's hand against the machine's fixed guard. The employee sustained an amputation.

An employee was transporting a patient to their residence. The employee bent down to grab a stretcher and felt a pop in their back. The employee sustained a herniation of their L4-L5 disc.

On November 18, 2023, at approximately 8:30 a.m., an employee was loading products onto a delivery truck. The employee lost footing and sustained a torn hamstring and a hematoma with nerve damage to his foot/leg.

An employee was carrying a pipe when it slipped from their hand. As they reached to grab it while it was falling, they felt a muscle pull. The employee was hospitalized for an abdominal hernia.

On August 21, 2023, an employee was reaching for product on the shelf in the cooler when their back gave out. The employee was hospitalized for an exacerbation of bulging disks.

An employee was vacuuming the inside of a used car when they sustained a lower back strain during the repetitive work, requiring hospitalization.

An employee was working to remove a spatula that got stuck in an auger at the bottom of a hopper. The employee went to remove the face plate to help release the spatula. His fingers were in the holes to pull off the face plate when the machine actuated. His right middle finger got trapped between the face plate and the inner plate, resulting in amputation.

An employee was walking in a parking lot during break when they were struck by a vehicle, resulting in fractures to their left scapula and lower right leg.

An employee bent down to turn on an air valve, lost their footing, and struck their hand on a pressure relief valve while trying to catch themselves. The employee suffered an amputation to their right index fingertip.

She retrieved a fallen cap for another production line from the floor and went to put in back in the machine it had fallen from (a capper). The machine caught the tip of her left little finger, causing a fingertip amputation.

An employee was taking apart the piping to a meat pump for clean-up. He saw food debris inside the pump housing and went to remove it. The meat pump rotor caught and severed his right index and middle fingertips, resulting in partial fingertip amputations.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
