An employee was unjamming the a machine when the maintenance operator began to assist in the process. The maintenance operator activated the machine and it cycled while the employee's hand was inside, resulting in the partial amputation of the left thumb and full amputation of the left index finger.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was walking down an exterior concrete stairway. She lost her footing, fell to the concrete, and suffered a compound fracture to the right ankle. She was hospitalized, requiring surgery.

An employee placed a box in the driving cage of a forklift to move it from one side of the warehouse to the other. As the employee approached the back portion of the area, the box shifted and hit the controls, causing the forklift to move toward the warehouse wall. The employee lost their balance, caught themselves, and stepped down off the forklift. The forklift continued and struck the wall, pinning their left leg/ankle against the wall. The employee moved the forklift forward and then fell to the floor, sustaining injuries to their leg and foot.

An employee was walking in the parking lot from their car to the front door when they slipped on ice and fell, resulting in an arm fracture.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
