An employee was returning to work from lunch. While talking to another employee on the other side of a closing access gate, the employee leaned against the gate before it completely stopped. His left hand was caught in the top roll bar of the gate, crushing the left fourth digit and requiring an amputation of the tip.
An employee was doing a repair in an office building. He was closing a bank vault door when his right index finger became caught between the door and the door frame resulting in amputation of the fingertip.
