A maintenance employee was working on a bus at the maintenance shop when he tripped and fell on a wheel chock. The employee was hospitalized with a fractured hip and required surgery.

An bus driver tripped in a parking lot where the angled concrete sidewalk meets the flat parking lot asphalt surface. She fell to the asphalt, suffering an injury to the left wrist that required surgery.

An employee was walking up steps into a school when she tripped and fell. At the same time, someone was opening a door to the school and she hit her face on the door. The employee suffered a shoulder injury and a neck fracture that required hospitalization.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee finished her afternoon shift and was walking to her car on the gravel/dirt/grass walking surface. As she approached the exit gate, she tripped/stumbled and fell onto a nearby fence and the ground. The employee sustained fractures to collarbones or rotator cuffs on each side of her shoulder(s).

An employee was standing in a doorway when she lost her footing, slipped, and fell to the floor, resulting in a broken elbow.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
