On April 30, 2025, a field service engineer was working on robots in a cell when the line faulted. The engineer went to unjam the line and the machine caused an amputation to their left index fingertip (about 3/4" total from the fingertip). The machine was not locked out/tagged out at the time.

An employee was driving a reach truck when it struck another reach truck that was backing away from a loading dock. The second truck came into contact with the employee's left leg through the open entrance of their own truck. The employee suffered a broken left femur and was hospitalized, requiring surgery.

An employee was performing maintenance on a stamping die and lowering the stripper plate back onto the die using a pendant on a hoist. The employee sustained a left hand fingertip amputation.

An employee was feeding slit coil into the press. As the employee cleared a jam, their gloves got caught in the material feeder resulting in a fingertip amputation.

An employee was cutting bar stock with an automated saw. The pneumatic clamp crushed her left ring finger at the nailbed. The fingertip was medically amputated at the first knuckle.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

An employee was cleaning a machine and bleeding its lines. After the gauge read 0, the employee removed the metal lid covering a filter. The lid popped off and struck the employee in the face, causing a broken nose, a broken jaw, and lacerations.

An employee was burned by an arc flash that occurred in an electrical room.

An employee was carrying a metal plate (18 inches x 18.75 inches) when it dropped onto his right foot, resulting in a fracture.

An employee was troubleshooting a variable frequency drive (VFD). There was an electrical discharge. The employee experienced an electrical shock of approximately 480 volts and sustained second- and third-degree burns on their right hand, wrist, forearm, and elbow.

An employee was operating a press and sustained broken bones in the right hand.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
