An employee was attempting to clear a pellet clog in a conveyor using a wrench when the hand wrench contacted the moving conveyor chain, causing the wrench to move forward and resulting in the employee's right index finger being pinched between the wrench and the conveyor housing. The employee sustained an amputation of the distal right index fingertip.
