An employee was using a table saw to cut plastic when the blade amputated the left middle and ring fingers.
An employee climbed up a portable stair ladder to grab a box from the shelf. On the way down, he missed a step of the ladder and slipped, falling and hitting the back of his head on the concrete floor. He suffered a head laceration and fracture.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
