A carpenter was operating the concrete mixer. He used an all-thread rod to loosen hardening concrete. The all-thread rod and the employee's shirt sleeve got caught in the concrete mixer's shaft injuring the employee's hand. He sustained lacerations to his fingers. In addition, one finger was amputated and another finger was partially amputated.
