An employee was in the process of switching railcars in the scrap yard when he slipped and fell from the engine landing beneath the railcar wheel as the railcar was moving. His right leg and left foot were crushed, resulting in a right leg amputation.

The pin slid sideways instead of sliding through the protector, pinching their left middle, ring, and little fingers between the pin protector and the end cap/disk of the rotor.
