An employee was installing satellite service at a customer's residence. While he was picking up a dropped screw from the grass, a snake bit his left middle finger.

An employee was using a forklift to fill orders in a warehouse and attempting to retrieve a pallet between two stacks of other pallets. As he was driving between the two stacks, his foot was caught on a pallet on the left side of the forklift, pinning his leg between the pallet and the forklift. His left leg broke, requiring hospitalization.

On August 4, 2020, an employee was installing a satellite dish at a customer's home. He fell off his ladder to the ground 20 feet below, suffering three broken vertebrae and a head injury. He was hospitalized.

On 8/08/2018, at 11:45 a.m., an employee was climbing a portable ladder when he fell to the ground and fractured his ankle, requiring hospitalization.

An employee was in a customer's attic performing installation work. At approximately 2:00 p.m., the employee began to feel ill and lightheaded. The employee was hospitalized for heat stress and low potassium.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

An employee had been installing an outside cable line at a residence. The employee became nauseous from working in the heat. The employee was hospitalized with heat exhaustion and dehydration.

An employee tripped at an elevation change while approaching a home. He fell to the ground and hit his head on a rock, suffering a contusion to the left eyebrow and a possible concussion.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
