Final narrative
An employee was in a trench box (near the ladder, about 5 feet inside the box) when a large boulder of dirt fell immediately outside the box. Some of the dirt rolled into the end opening of the box, striking the employee on his heel as he tried to get away and knocking him down to the ground. As he fell, his head struck a T-valve, resulting in contusions to his head and face area. He was hospitalized.

The injured employee was in a 7- to 8-foot deep trench installing a large, 66" diameter reinforced concrete pipe. An excavator bucket came loose and fell into the trench, striking the employee on the left side of their body. The employee sustained a large contusion on the upper left leg, a broken left arm, and their left hand was surgically amputated.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.
Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.
