An employee was using an overhead hoist to lift an assembly (tire and rim) that was still attached to a mounting machine. When the employee lifted the assembly, it caused the machine to lift as well, so the employee unlocked the assembly. At that time, the mounting machine fell on the employee's left foot, crushing it.
