An employee was using a bolt gun when the bolt shot his temple and he sustained a head injury.

Employee was preparing to euthanize a non-performing hog. He went to the shop and retrieved a Captive Bolt Gun and returned to the pen where the hog was located. He loaded the gun and put the safety on. As he was climbing into the pen, the gun fell. When it hit the floor, the gun discharged, and the 3" projectile attached to the gun struck the caregiver in the right thumb, puncturing his thumb and breaking the bone.

Burton Pools and Spas

An employee was helping to install a new liner in a swimming pool. The employee entered the pool and stepped into a small uneven surface under the liner. The employee's right ankle twisted outward, resulting in a sprained ligament and a nondisplaced fracture to the right ankle.

Engineered Floors, LLC

An employee was looking for defects in a carpet while standing on the floor between a catwalk and the exit roller of a tufting machine. He stepped backward and tripped on the edge of the catwalk, grabbing onto the exit roller of the machine with his left hand in order to catch himself. While doing so, his left index finger went into the keyway hole on the rotating exit roller, which caused his left index finger to be lacerated.

An employee was delivering mail when she stepped on a board which broke, causing her to twist and fracture her ankle.

Toyo USA, Inc.

An employee was walking down concrete steps from a 19" concrete platform. On the last step, the employee's left foot stepped on the front edge of the step, causing the employee's right foot to roll. He lost his balance and continued to step down. His body and leg moved in a twisting motion, causing a break in his right tibia as his right foot impacted the ground. He did not fall down. He then sat down on the step and radioed for help.

An employee was holding a barn door open while putting a tool away. The door shut on the employee's right hand, causing a partial amputation to the little finger.

Platte Center West

An employee was using a bolting device to euthanize pigs when he slipped and fell. The bolting device pierced his pelvic area and the employee's femur was fractured. The employee was hospitalized and required surgery.

JBS Live Pork, LLC

A mature sow in the gestation barn struck a gate and the gate swung around, striking an employee's right ankle and causing a crush injury.

An employee was moving a dead animal using a removal cart. While transporting the animal, the cart tipped over and landed on the employee, resulting in a tibia fracture.

An employee was helping remove a dead animal from a barn. A truck was backing up to the barn to retrieve the body, and its lift platform pinned the employee's leg against the barn. The leg was severely lacerated and the employee was hospitalized, requiring surgery.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Meeco Sullivan

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

Edward Jones Investments

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

Burgess Manufacturing of Oklahoma Inc.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
