An employee was surveying for plants in a canyon. While hiking through the canyon, a rock dislodged and stuck the employee on the left ankle and foot. The employee sustained fractures to the ankle and foot.

An employee was crouched down, doing maintenance work on a brush cutter, when a truck backed over her leg. Her right femur was broken.

Performance Contractors, Inc.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

WIRECO WORLDGROUP, INC.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

Owens & Minor - Pittsburgh Distribution Center

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Retro Tech Systems

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

Creston Energy Group

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

ISO TECHNOLOGIES, INC.

A temporary employee climbed onto a 28-inch high table to clean the top of the head assembly of the profile saw used to cut polyurethane foam. While cleaning, he tripped over the pneumatic hose and struck the blade. He then fell over to the other side of the head assembly onto the table. The employee sustained an amputated right foot and a laceration on his left leg. The machine was not locked out/tagged out and guarding was not used at the time.

MIT LINCOLN LABORATORY

An employee was sitting in an office chair when his foot caught on one of the rollers. He fell from the chair to the floor and suffered a broken hip. He was hospitalized.

BAE Systems Inc

An employee was disconnecting a resistor in an electrical test setup and contacted an energized power terminal. The employee suffered an electric shock and was hospitalized.

An employee fell into a door and the door broke. He then fell to the tiled floor. The employee sustained fractures to their left hip and femur.

An employee was walking down the hallway when they tripped over a raised concrete slab on the floor and fell. They sustained a right elbow fracture that required hospitalization.

Department of Justice - USMS

The injured employee was assisting a Department of Homeland Security (DHS) enforcement operation. During an apprehension, officers returned fire at a subject and struck the employee with a bullet. The employee sustained a gunshot wound to their right hand.

M1 Support Services

The injured employee was assisting in the disassembly phase after a radar array had been lowered and secured. The crew began removing load bearing pins from an overhead crane to free the radome. A load bearing pin was stuck. The injured employee went to remove the pin manually as a second team member applied pressure from the opposite side. The pin unexpectedly released and struck the injured employee s right thumb, resulting in partial amputation of the distal phalanx including an open distal phalanx fracture and nail bed laceration.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

Dept. of Homeland Security - CBP

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

Sycuan Casino Resort

An employee was preparing a bundle of green onions for chopping. While holding the bundle in his left hand, he made his first cut using a 9-inch kitchen knife held in his right hand. The knife contacted the tip of his left thumb, resulting in an amputation of approximately 0.5 inches of the thumb that required hospitalization.
