An employee was installing a new damper actuator on an air handler unit. He was using a cordless impact driver while working from a step ladder. While driving a self-tapping screw into a bracket, the screw bound up in the bracket causing the bracket to spin. The bracket knocked off the employee s prescription safety glasses and struck his right eye.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel.
