An employee was assisting in attaching a bucket, with a large pin, to an excavator. The pin was dropped and it crushed the injured employee's hand against the bucket. The employee sustained the amputation of the ring finger and was hospitalized with open fractures to multiple fingers of the left hand.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
