An employee was inserting a drain plug underneath a macerator machine. The drain plug slipped from the employee's hand, and the employee's middle finger went into the drain hole and contacted the spinning auger blade. The fingertip was amputated. The machine was not locked out at the time.

An employee was washing down an area using an 8-foot portable ladder. The employee was descending the ladder and on the way down, felt dizzy, missed the last step, and fell and hit his head on the concrete floor. The employee suffered a cut on the scalp that required stitches and hospitalization.

The mower started running hot, so he brought it back to the shop and opened the hood on the mower. The radiator overflowed the bottle, the lid blew off, and hot coolant sprayed the employee's face, eyes, and neck. The employee was hospitalized for burns.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
