The patient was able to place his hand on the weapon and it fired while still inside its holster. The injured employee was struck in the abdomen by a bullet while standing at the door opening. The employee was hospitalized.

An employee attending valeted vehicles was struck by a customer's car, which was parked without the brakes engaged. The employee suffered a back injury.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
