An employee was cutting scrap metal when their shirt caught on fire. The employee sustained burns to the middle of his back and was hospitalized. Flame-resistant personal protective equipment was not used.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
