A service technician was tasked with moving equipment. As he was moving a carrier basket from a pallet, the equipment fell and crushed his right index finger. The employee was taken to the hospital where the tip of his right index finger was amputated.

A crew was dismantling a rig. They were closing the split spider after removing it from the casing when the injured employee's left hand was caught between the split spider and the rig's master bushing, which was also on the rig's floor. The employee suffered a laceration to the middle finger and fractures to the middle and ring fingers.

An employee was on the rig floor standing on the off driller's side at the stump to help run pipe casing. As the next joint was being brought in, the employee's finger was caught between the stump and the projector on the joint. The employee's right ring finger was amputated.

Two shop employees were in the process of tearing down a 1250 Ton Slip Type Elevator (STE) and were pulling the carriers and slips out of the STE. While removing one of the carriers from the slip, utilizing the overhead crane, the entire slip assembly (carrier, slip, and slip hanger) began to rise out of the tool. As the slip hanger rose the IP reached in to remove it when the slip suddenly released from the carrier pushing the slip hanger back into position. The IP's finger was caught between the slip hanger and the timing ring of the STE amputating the tip of his left index finger midway up the fingernail.

Employees were making up float shoes on the first joint of a casing job. The pipe came loose, swung, and caught one of the employees between the manual back-up tong and the back-up snub line. The employee was hospitalized and treated for injuries to the chest and colon area.

An employee was trying to move a pressure vessel when it fell over. A stud on the top of the vessel struck and gouged the employee's stomach.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.
