An employee parked a commercial motor vehicle at a truck stop. The employee slipped while exiting the vehicle and fell to the ground. The employee was hospitalized with a fractured left distal humerus.

An employee was releasing the 5th wheel level on the hitch when the parking brake was released. The truck tire ran over the employee's foot causing open lacerations and fractures.

An employee was in the rear of delivery truck unloading rolling metal cages containing merchandise. As the employee lowered a container to the ground with a lift gate, it began tipping over. The employee extended his right arm to prevent the load from falling and his right little finger was caught between the rolling cage and the trailer door frame. He sustained an open fracture to the right little finger.

An employee was lowering a wheeled, steel cage filled with merchandise from the tailgate of a truck. When the tailgate made contact with the ground, the cage shifted and rolled off the gate, striking the employee's left ankle and resulting in a fracture that required hospitalization and surgery.

An employee was unloading a truck using a cart. The cart tipped over onto the employee's leg and fractured the left tibia.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
