An employee was on a ladder, gathering materials from a shelf. She slipped and fell from the ladder to the concrete ground, suffering a broken heel and foot.

At approximately 21:00 CDT, on July 22, 2025, an employee was doing priority pulls in the backroom (storeroom). She ascended a track ladder attached to an aisle to retrieve an item of clothing. On her way down, she missed a step or two and fell to the concrete floor. The employee sustained a fractured left ankle.

An employee was carrying paper products in an aisleway, tripped over one of the packets they were carrying, and fell to the floor resulting in a lower left leg fracture.

While pulling items from a shelf, an employee slipped and fell from a track ladder onto the concrete floor on her left leg. The employee was hospitalized for a leg fracture that required surgery.

An employee fell from a ladder while taking something from a storage shelf. She suffered a broken knee.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee tripped over a chair in the break room and fell, resulting in lower back pain. The employee was hospitalized with a fractured vertebra.

An employee was performing a pre-operation assessment of an electric pallet jack. The pallet jack activated and pinned the employee's left leg against a guardrail, breaking the tibia. The employee was hospitalized.

An employee was standing on a pallet when their foot caught on the pallet and they fell to the concrete floor, resulting in a fractured right femur.

An employee fell from a chair to the floor, suffering a broken right leg/ankle. She was hospitalized.

An employee fell to the floor in a store, suffering a broken right femur, a contusion and inflammation under the right eye, and a contusion and inflammation to her left wrist and forearm.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
