An employee was changing out the die on a pellet mill. When he spun the die, it caught his left-hand thumb, resulting in the amputation of 1/2 an inch of his thumb.

Kreamer Feed, Inc.

An employee was operating a forklift when the lift stopped working in the middle of the driveway that leads from the receiving area to the office. The employee used a skid loader and a towing strap to pull the inoperable forklift. The strap broke causing the forklift to start rolling downhill (approximately a 10% incline). The forklift rolled approximately 30' before one of the forks caught the railroad track, causing it to turn toward the injured employee who was standing in that area. Unable to get out of the forklift's path, the injured employee grabbed the forklift mast and started running backward holding onto the lift. The lift traveled approximately 12' with the employee holding onto it before it rolled down an embankment (approximately 28" deep), trapping the injured employee between the lift and the bank. The employee was hospitalized with a fractured pelvis.

N. S. Giles Foundations, Inc.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

Weld Mech, Inc.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

He was using a hydraulic tool to pull the pieces apart when a bolt dislodged from the eye of the gearbox and the corner of the gearbox broke off. The employee was struck in the nose (sinus cavity), resulting in hospitalization and surgery.

An employee was on a forklift checking the sprinkler system drain. The employee fell 8 feet from the forklift to the floor. The employee suffered a broken collar bone and was hospitalized.

An employee was working as a dumper on a packaging line. A passing forklift turned and pinned the employee against a guardrail. The employee suffered fractures to their ribs and a leg requiring hospitalization.

Gorman Milling Co. Inc.

An employee was cleaning an upper mezzanine. When he leaned against the mezzanine gate, he fell through the gate opening and landed 11 feet below. The employee sustained fractured orbital bones.

Sanderson Farms LLC, Production Division

An employee was assigned to clean a feed mill in the basement. He then rode a tool and material basket to return from the basement to the ground level. He fell out of the basket and landed about 5 feet below. The employee sustained fractures to his arm, lower back, and foot.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
