An employee was lining a hydraulic cylinder up with the eye (opening). The hydraulic cylinder was attached to a 300-pound crush plate and was supported by a pry bar. The pry bar slipped out and the cylinder dropped, pinching his right middle finger and causing a fingertip amputation.
