On June 16, 2017, at approximately 10:50 p.m., an employee operating a terminal tractor was preparing to hook up airlines to a trailer. He exited the back door and stepped onto a catwalk made of metal grating when he lost his balance and fell between the catwalk and fuel tank. He fractured his left shoulder and was hospitalized.

An employee entered the cargo area on an aircraft to inspect the ceiling under the wing when they stepped into an open hole from a missing aircraft floor board. The employee twisted his right ankle, resulting in a dislocation.

An employee was walking from a job trailer to a delivery truck when he stepped into a hole and heard a pop in his right knee. The employee was hospitalized for an injury to the right knee.

An employee carried an extension ladder to the work location and was raising it to a strand. He realized that the strand hooks were not extended, so he slowly lowered the ladder to extend the strand hooks. He then stepped into a hole in the customer's yard and twisted his ankle. He was hospitalized with a fractured ankle and had surgery.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.
