The machine grabbed the tool and the employee's left arm was pulled into the machine. The employee was hospitalized and a surgical procedure was performed.

An employee was standing at the administration station counting parts on pallets, verifying correct parts are being sent, and entering shipping information into the computer program. A forklift struck a pallet of approximately 80 cabinet base panels and the panels fell onto the employee who suffered a broken left tibia.
