An employee was using a line up bar, which is a metal bar with tapered ends. The tapered ends are inserted in the holes of a frame in order to align the frame. While inserting the bar in the holes, one of the ends broke off and struck the employee in the left eye.
A changeover was being performed on a pleater, and an employee was helping feed media into the machine's rollers. A roller pinched the employee's fingers resulting in amputation of a finger. The machine was not locked out at the time.
